Your organization can create custom foster forms to collect the specific information you need. Custom forms replace the standard foster form with your own fields and sections.
What Are Custom Forms?
Custom forms let you:
- Add fields specific to your organization
- Remove fields you don't need
- Organize information into sections
- Make certain fields required
Using Custom Forms
When your organization has a custom foster form:
- You'll see different fields than described in other documentation
- Fields will be organized into your custom sections
- Required fields will have a red asterisk (*)
How to Tell You Have Custom Forms
Your organization has custom forms if you see customized fields when adding or editing a foster. You can check available form templates by going to Settings > Forms > Foster tab.
Filling Out Custom Forms
- Work through each section
- Fill in all required fields (marked with *)
- Follow any field instructions
- Click Add Foster or Save Foster
Field Types You Might See
Text Fields
- Text - Single line for names, short answers
- Textarea - Multi-line for notes and descriptions
- Email - For email addresses with validation
- Tel - For phone numbers with formatting
- Address - With autocomplete functionality
Choice Fields
- Select - Dropdown for single selections
- Multiselect - For multiple selections
- Boolean - Yes/No button selection
- State - US state dropdown
Other Fields
- Date - Calendar picker for dates
- Number - For numeric values
- Agreement - Terms acceptance with optional signature
Viewing Custom Form Data
When viewing a foster profile created with a custom form:
- Information is organized by your custom sections
- All submitted data is displayed
- Empty fields show as "-"
Editing Custom Form Profiles
- Click Edit on the foster profile
- The custom form loads with current information
- Make your changes
- Click Save Foster
Public Foster Applications
If you accept online applications:
- The public sees your custom form
- Applications come in as "pending" status
- Review and approve/decline from the foster profile
Important Notes
- Custom forms are set up in Settings > Foster Form
- Only admins can create or modify form templates
- All fosters use the same form template
- Data from custom fields appears in the foster profile
